1) was this arranged through gamefinder
2) did you have resets/disconnections or did you enter and exit the game because of other reasons

Think this is fixed in test division, just been a while since we got it all stable and could push to live.

"fixed" is a strong word in this context. Since we can't reproduce it we can't build a definite fix. But there is a change that might fix this by removing a possible cause. That's as good as it gets atm..
